In this training, we will explore how to create and maintain the core of the counseling relationship: the therapeutic alliance. It is this connection between client and clinician that has such a large influence on outcomes in therapy. Together, we will examine the nature of rapport and discuss what strengthens and weakens it. We will also discuss common barriers that prevent us from fully connecting with clients, and how to successfully address those obstacles.

Upon completion of this training, participants will be able to:

  • Define the term “therapeutic alliance”
  • Identify how the therapeutic alliance influences therapeutic outcomes
  • Determine strategies for strengthening the therapeutic alliance
  • Describe how certain actions may weaken or destroy the therapeutic alliance

Syllabus:

Objective #1: Defining the Therapeutic Alliance

  • Describe the therapeutic alliance as a collaborative relationship between clinician and client
  • Identify core components including communication, trust, empathy, honesty, and respect
  • Understand rapport as the foundation of connection and trust in the therapeutic space
  • Explore personal definitions and experiences of therapeutic connection

Objective #2: The Influence of the Therapeutic Alliance on Clinical Outcomes

  • Review research findings linking strong alliances to positive therapeutic outcomes
  • Emphasize that the quality of the relationship outweighs technique or theoretical model
  • Reference key works such as The Heart and Soul of Change

Objective #3: Strategies for Strengthening the Therapeutic Alliance

  • Cultivate self-awareness, mindfulness, and curiosity about the client’s experience
  • Build relationships through collaboration, empowerment, and shared goals
  • Use effective communication including verbal and nonverbal cues
  • Demonstrate emotional responsiveness, empathy, and consistent follow-through
  • Practice reflective listening, transparency, and appropriate flexibility
  • Apply Carl Rogers’ principles of congruence, empathy, and unconditional positive regard
  • Enhance connection through attention to client “touchpoints” including physical space and intake processes
  • Consider cultural, social, and identity-based influences on client-clinician dynamics
  • Integrate awareness of transference, countertransference, and power dynamics
  • Use the Minority Stress Model to inform culturally sensitive engagement

Objective #4: Actions That Can Weaken or Disrupt the Therapeutic Alliance

  • Avoid overwhelming or impersonal intake procedures
  • Refrain from judgmental or biased communication
  • Recognize the harm of selective listening or rigid treatment preferences
  • Be cautious about overreaching interpretations or dismissing client input
  • Minimize distractions such as clock-watching or poor session pacing
  • Avoid technical jargon and disconnection from the physical setting
  • Monitor body language and behaviors such as yawning or forgetting key client details
  • Understand signs of relational rupture and learn how to initiate repair
  • Respect client autonomy while remaining attuned to signs of insecure attachment

Social workers completing this course receive 1.5 Clinical asynchronous continuing education credits.

Course Instructor: Dreya Blume is a licensed clinical social worker, with her MSW from Radford University and an MA in anthropology from the University of Hawaii. Dreya has twenty years of experience working in mental health, from case management to intensive in-home services to leading groups for addicts in recovery. Dreya began a part-time counseling practice in 2004, and as a psychotherapist, her focus has been on serving the LGBTQ+ community. Dreya also has been a teacher for thirty years. Prior to her career in social work, she taught English in places such as Japan, Thailand, Australia, and Hawaii. Since 2010, Dreya has been an adjunct professor for Virginia Western Community College, preparing future social workers for the field. In her spare time, Dreya loves to write. She is the author of a number of books (published under her former name, Andy Matzner), including “Everyone Has a Story: Using the Hero’s Journey and Narrative Therapy to Reframe the Struggle of Mental Illness,” “The Tarot Activity Book: A Collection of Creative and Therapeutic Ideas for the Cards,” and “The Buddha Diet: A Guide to Creating a Positive Relationship with Food and Eating.”
